If you are a health care professional, community representative, potential client or family member please contact us through the following form. This is not for any emergencies, as they should be directed to the appropriate 911 service channels. If you are a referring source please include both your name and phone number, as well as the client's/ patient's name, phone number and address. The comments section can be used for details.

Home Health Aides, Nurses and Other Caregiver's

If your doctor decides you need home health care, you have the freedom to choose among a list of participating Medicare-certified home health agencies that serve your geographic area to give you the care and services you need. Your choice should be honored by your doctor, hospital discharge planner, or other referring agency. When the hospital discharge planner or your doctor recommends home health care for you or your loved one, ask for care from Golden Age Home Care.

Section 1802 of the Social Security Act seeks to ensure that free choice is guaranteed to all patients. The law states: “Any individual entitled to insurance benefits under this title may obtain health services from any institution, agency or person qualified to participate under this title if such institution, agency or person undertakes to provide him such services.”

This statement gives patients freedom to choose whom they want as their provider of home health services.
